Going forward, Columbia Park Medical Group will require your insurance card for all calls and visits. Please have your card ready.
Please allow adequate time to receive and process your scheduling request.
Due to possible time delays, all of the following must be requested by calling (763) 572-5700: message(s) to your provider (option 5) and same day appointments (option 4).
To confirm your appointment, you will receive a confirmation call or an email reply from a Scheduling Representative within one business day.
Please note: if you are using a pop-up blocker, you may need to allow pop-ups for this site to complete the appointment request forms.
Same day appointments may be requested by calling (763) 572-5700 (option 4).
CPMG provides Urgent Care on a walk-in basis, No appointment necessary.
